Print the document. Then, take a pen or pencil and fill out the form. There are 3 sections with 20 questions you should answer YES or NO to, and there’s also a place where you should keep track and count the number of all your “YES” answers.

This number tells a story about the current condition of your company when it comes to sales revenue growth, so please pay attention. Reflect on your answers, especially the ones where your answer was “NO”.
